Using insurance

Benefits vary by plan.

Participation with an insurer or platform does not guarantee that every service is covered. Deductibles, copayments, coinsurance, authorization requirements, and exclusions may apply.

We may request a copy of your insurance card and date of birth to verify available benefits. Verification is an estimate—not a guarantee that your plan will pay a claim.

Self-pay

Ask before scheduling.

Self-pay fees are disclosed during the scheduling process. The amount depends on the service and appointment type. Laboratory testing, prescriptions, supplements, and services provided by outside companies are billed separately unless stated otherwise.

Uninsured or self-pay clients may request a Good Faith Estimate of expected charges.

The laboratory handles test billing. Coverage and out-of-pocket cost vary; Thrivewell cannot guarantee the amount owed.

Late cancellations

Late-cancellation and missed-appointment charges are governed by the practice policies provided before care and are generally not covered by insurance.

Records and administrative services

Fees may apply to certain non-clinical requests, forms, letters, or services not covered by insurance. Availability is subject to practice policy.
