Reliable care depends on clear expectations.

The summaries below provide general information. Clients receive the complete policies, consents, and financial agreements that apply to their care.

Appointments and attendance

Appointments are reserved for you. Cancellation, rescheduling, late-arrival, and missed-appointment requirements are explained in the practice agreement. Fees may apply when notice is not provided within the required timeframe.

Telehealth location

You must be physically located in a jurisdiction where your clinician is authorized to practice at the time of the appointment. Your identity, location, and emergency contact information may be verified.

Communication

Routine messages are answered during business hours. Email, voicemail, and portal messages are not continuously monitored and must not be used for emergencies. Do not include sensitive clinical information in ordinary email.

Prescriptions and refills

Refills require appropriate follow-up and monitoring. Requests should be made during business hours with sufficient notice. Prescriptions, controlled medications, early fills, replacement prescriptions, and medication changes are never guaranteed.

Forms, letters, and records

Requests are evaluated individually. Some forms, legal opinions, disability determinations, or administrative services may be outside the practice’s scope or require an established treatment relationship, adequate notice, and a separate fee.

Ending or transferring care

Care may be transferred when clinical needs exceed the practice’s scope, required monitoring is not completed, policies are repeatedly not followed, or another setting is more appropriate. Transition information is provided when clinically and legally required.
