• Initial certification or renewal when Maryland requirements and clinical criteria are met

POSSIBLE QUALIFYING CONDITIONS
Maryland recognizes certain chronic or debilitating conditions, including post-traumatic stress disorder, severe or chronic pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, cachexia, anorexia, wasting syndrome, and other severe chronic conditions for which other treatments have been ineffective.

• Relevant medical records and your Maryland Cannabis Administration patient information may be required
• Appointment location and format must comply with current Maryland requirements
